HEALTH AND LIFE BENEFITS
Benefit: MEDICAL INSURANCE
Eligibility:
- Regular status employees working a minimum of 24 hours
a week.
- Medical insurance is effective the first day of the
month following three months of employment.
Description:
There are four plans for the employee to select from:
- PHCO High Option Plan
- PHCO Value Option Plan
- Group Health Cooperative - Co-Pay Plan
- Group Health Cooperative - Welcome Plan
The hospital pays 100% of the premium for the employee’s coverage for three of
the plans. The employee is responsible for 100% of the premium through pre-tax
deductions to add spouse and/or dependents.
Benefit: DENTAL INSURANCE
Eligibility:
- Regular status employees working a minimum of 24
hours a week.
- Dental insurance is effective the first day of the
month following three months of employment.
Description:
There are two plans for the employee to select from:
- Washington Dental Service - Delta Dental Plan
- Willamette Dental of Washington
The hospital pays 100% of the premium for the employee’s coverage for one of
the plans. The employee is responsible for 100% of the premium through pre-tax
deductions to add spouse and/or dependents.
Benefit: EMPLOYEE ASSISTANCE PROGRAM
Eligibility:
- Regular status employees working a minimum of 24 hours
a week.
- Employees are eligible the first of the month following
3 months of employment.
Description:
The hospital pays 100% of the premium for this confidential employee assistance
program designed to assist employees and family members with work-related,
personal and family problems. Services provided include: counseling, referrals
to community resources, group and family therapy. Employees and family members
receive three free sessions per incident and an unlimited number of informational
telephone calls.
Benefit: LIFE INSURANCE AND ACCIDENTAL DEATH & DISMEMBERMENT
Eligibility:
- Regular status employees working a minimum of 35 hours
a week.
- Employees are eligible the first of the month following
3 months of employment.
Description:
The hospital pays 100% of the premium. The employee’s annual salary is doubled
and rounded to the next $1,000. The maximum benefit is $300,000. Dependent
benefit is $1,000. Additional coverage may be purchased for employee, spouse
or children.
Benefit: ADDITIONAL ACCIDENTAL DEATH & DISMEMBERMENT
Eligibility:
- Regular status employees working a minimum of 16 hours
a week.
- Employees are eligible the first of the month following
3 months of employment.
Description:
Employees may elect to purchase additional coverage for themselves and their
family. Premiums range from $1.18 for $25,000 to the maximum of $17.25 for
$250,000. The spouse will receive 50% of the employee benefit and child(ren)
will receive 15% of the employee benefit.
Benefit: LONG TERM DISABILITY
Eligibility:
- Regular status employees working a minimum of 35 hours
a week.
- Employees are eligible the first of the month following
3 months of employment.
Description:
The hospital pays 100% of the premium. Employees receive 66-2/3 of their monthly
salary after 180 days of continuous disability to age 65.
Benefit: SHORT TERM DISABILITY
Eligibility:
- Regular status employees working a minimum of 24 hours
a week.
- Employees are eligible to enroll following 3 months of employment.
Description:
The employee pays 100% of the premium through payroll deduction. Plan information
and enrollment packet will be mailed to employee's home when eligible to enroll.
Benefit: LONG TERM CARE INSURANCE
Eligibility:
- Employees are eligible upon employment.
Description:
This benefit may be purchased by the employee to cover themselves, spouse, parents,
parents-in-law, grandparents and grandparents-in-law, and adult children.
LEAVE BENEFITS
Benefit: SICK LEAVE
Eligibility:
- Employees are eligible to use their sick leave after
completing three months of continuous service and are paid on the first day of
the illness or injury.
Description:
Sick leave hours accrue based on the actual hours worked by the employee:
- Regular full-time employees accrue sick leave hours
at 8 per month up to a maximum of 1,000 hours.
- Regular part-time employees accrue sick leave hours
on a prorated basis up to a maximum of 1,000 hours.
Benefit: VACATION
Eligibility:
- Regular status employees are eligible to use their
vacation benefit after six months of continuous service
Description:
Vacation hours accrue based on the actual hours worked by the employee:

Benefit: PAID HOLIDAYS
Eligibility:
- Regular status employees are eligible upon employment.
Description:
Holy Family Hospital recognizes the following six paid holidays: New Year’s Day,
Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas.
If a non-exempt employee works on a holiday he/she will be compensated at the
rate of time and a half and, in addition, will receive a paid day off 30 days
before of after the holiday.
Part-time employees receive holiday pay on a pro-rated basis.
Benefit: PERSONAL HOLIDAYS
Eligibility:
- Employees are eligible to use their three personal
holidays after completing a 90-day probationary period and according to the schedule
described below.
Description:
- Employees who are hired on or after March 1 may
use two personal holidays in their first year of employment.
- Employees who are hired on or after September
1are not eligible to use personal holidays in their first year of employment.
Schedule:
- Personal holiday one………………..January-June
- Personal holiday two………………..July-December
- Personal holiday three………………January-December
- Part-time employees receive personal holiday pay on
a pro-rated basis.
Benefit: BEREAVEMENT LEAVE
Eligibility:
- Regular status employees are eligible upon employment.
Description:
An employee on regular full or part-time status may receive up to 40 hours of
paid absence in the event a death occurs in the immediate family. Immediate
family is defined as: spouse, child, brother, sister, parent, grandparent grandchild,
parent of spouse, brother of sister of spouse, domestic partner or any living
dependent in the household of the employee.
Part-time employees receive bereavement hours on a pro-rated basis that is determined
by the number of hours they are normally scheduled.
Benefit: GENERAL LEAVE OF ABSENCE
Eligibility:
- Regular status employees are eligible after completing
six months of continuous employment.
Description:
A leave of absence may be granted for personal reasons, education, extended illness
or disability supported by a doctor’s statement indicating inability to work
emergencies or other compelling circumstances.
Leave for military service or military reserves shall be granted pursuant to
applicable federal and state law.
Employees may be on a general leave of absence for a maximum of six months unless
approved by Administration. If granted, a leave of absence will protect the employee’s
accrued benefits and seniority. The privilege of returning to work, however,
is subject to employment conditions at the time of return.
Vacation, paid sick time and holidays will not accrue while an employee is on
unpaid leave of absence status. Group life and medical insurance benefits will
continue up to six months during an approved leave of absence at the employee’s
expense.
Benefit: MATERNITY LEAVE
Eligibility:
- Employees are eligible upon employment.
Description:
Maternity leave is an unpaid job-protected leave for 6 weeks or for a physician
verified period of disability related to childbirth. If the employee elects
to take leave for longer than the period of disability, the employee’s return
to active employment will be subject to any applicable federal or state law,
employment conditions, and available positions at the time of return.
During the time period an employee’s physician states that the employee is unable
to work, the employee may use accrued sick leave hours. Unpaid maternity leave
may be requested to cover the period of disability if the employee exhausts accrued
sick hours.
Benefit: FAMILY AND MEDICAL LEAVE
Eligibility:
- An employee must have worked for at least one year
and 1,250 hours over the previous twelve months.
- Under the Family and Medical Leave Act of 1993 (FMLA),
an employee may qualify for leave for the following reasons:
1. the birth and care of a newborn child.
2. the placement of a child with you for adoption or foster care.
3. the need to care for an immediate family member (spouse, child, or parent) with a serious health condition.
4. a serious health condition that makes you unable to perform the essential functions of the job.
Description:
Family and medical leave provides up to 12 weeks of unpaid job-protected leave.
Holy Family Hospital will maintain the employee’s health coverage as if the
employee were still working and, the employee will, in most cases, be returned
to the same position held when the leave commenced or a comparable position.
Accrued sick leave and vacation time will be used at the beginning of the FMLA
leave allowing for all or a portion of the leave to be paid.
Benefit: JURY DUTY
Eligibility:
- Employees are eligible upon employment.
Description:
The hospital pays the difference between the employee's regular salary and the fee received for jury duty.
EMPLOYMENT BENEFITS
Benefit: RETIREMENT PLAN, 403(b) (Employee Contributions)
Eligibility:
- Employees are eligible upon employment and may enroll
in the plan at any time.
Description:
The 403(b) Retirement Plan is a Tax Deferred Annuity (TDA), that is; employees
may contribute through pre-tax deductions up to $15,500 (2007) annually. Catch-up
provision for employees over 50 years of age.
Benefit: RETIREMENT PLAN, 401(a) (Employer Contributions)
Eligibility:
- Employees are eligible when they are contributing
to the 403(b) component of plan, and after working minimum of 800 hours within
Plan year, and be considered active on the final day of the last payroll period
of the year.
Description:
Employer match starts at .50 cents for every dollar up to the first 3% of pay
an employee contributes. Matching increases with continued years of service,
up to .50 cents for every dollar up to the first 6% of pay an employee contributes.

PLAN (Employer Matching Contributions)
Eligibility:
- Employees are eligible after working minimum of 800
hours within Plan year, and employee is at least 21 years of age.
Description:
Holy Family makes an annual contribution to your account, equal to 5% of your
pay, and your account is also credited with interest at an annual rate that
is fixed for the year.
Benefit: WORKERS' COMPENSATION
Eligibility:

Description:
Holy Family Hospital is a self-insured employer that provides Worker's Compensation Insurance for all employees. Holy Family Hospital pays the actual cost of medical charges and time loss compensation expenses for all employees.
Benefit: FLEXIBLE SPENDING ACCOUNT
Eligibility:
- Regular status employees working a minimum of 24 hours
a week.
- Employees are eligible the first of the month following
3 months of employment.
Description:
Employees may elect to have a specific pre-tax amount deducted from their paycheck
and set aside to pay for insurance premiums, medical/dental expenses not covered
by insurance (copays, deductibles, braces) and dependent care expenses (childcare).
Any money not used in the designated period will be forfeited.
Benefit: TUITION REIMBURSEMENT
Eligibility:
- Employees are eligible the first of the month following
12 months of employment.
Description:
The hospital will pay 50% of approved tuition and books up to:
- $150 annually for part-time employees, 8-23 hours a week
- $250 annually for part-time employees, 24-34 hours
a week
- $500 annually for full-time employees, 35 or more
hours a week.

Description:
- Inservice and continuing educational programs are offered inside
and outside the hospital throughout the year. Forms are made available in each
department
and employees are encouraged to apply.

All employees wearing a Holy Family Hospital name badge will receive a 10% discount in the hospital cafeteria. Employees may also sign up for the cafeteria debit plan.

Description:
Paychecks are mailed to the employee's home address.
Employees may elect to have their paycheck deposited directly into their bank account, then the paycheck receipt is mailed to the employee's home address.

Eligibility:

Description:
This fund was created to assist employees who may be experiencing financial crisis. Fellow employees may contribute to the fund with a payroll deduction each pay period.
Benefit: AMICUS FEDERAL CREDIT UNION
Eligibility:

Description:
Employees may join the credit union and do not have to pay a membership fee.
Once a member, employees may elect to deposit into a savings account at the credit union via payroll deduction.

Description:
Holy Family Hospital sponsors four blood drives per year. Employees are only required to donate once a year. The Family Blood Plan will cover the blood needs of the employee and family.
